CONTENTS

The use of piezo surgery through a full open approach has revolutionized rhinoplasty, by allowing a complete visualization of the nose. The anatomic variations of the bones can be assessed and addressed in an adapted way. Bones can be precisely moved and orientated, but also sculpted and reshaped. Bone stability is usually preserved, but it can also be reestablished if this stability is weakened. The keystone area is perfectly controlled and smoothed throughout the whole procedure. The septum can be very precisely reshaped or harvested whatever its characteristics and deviations, minimizing the risks of imperfection after septoplasty. This perfect control on bone fracture lines and septal trimming allow to easily perform middle third conservative techniques (push down/let down). Finally, rig graft harvesting is risk less and fast with the use of piezo instruments. Ultrasonic rhinoplasty through a full open approach is easy and reliable. The current techniques developed since 2013 are detailed through videos, with an extensive explanation of the instruments use, but also on the techniques sequences and on the management of difficulties.

OBJECTIVES

  • Understand what is piezo surgery: effects, advantages and drawbacks with other techniques of osteotomies.
  • Get familiar with the piezo instrumentation (Rhinoplasty inserts).
  • Understand the techniques of osteotomies, ostectomies, bone sculpting, bone smoothing, their sequence and their indications regarding the width and shape of the bony vault.
  • Know when to do partial or complete osteotomies.
  • Learn how to do an open push down/ let down technique.
  • Learn how to do osteosynthesis to stabilize bones or to treat bone defects.
  • Understand the different management of the keystone area achieved with piezo instruments.
