■ Ferreira-Ishida or Spare-Roof Technique B for dorsum reduction, crooked nose, saddle nose and augmentation rhinoplasty

CONTENTS
The Spare Roof Technique (SRT) is a middle third preservation technique, described in 2016 by Ferreira MG et al., that allows the surgeon to isolate the cartilaginous roof of the middle third without splitting the Upper Lateral Cartilages (ULCs) among them.
In 2020 the author starts the SRTb – thinking that it would be possible to dehump the nose while preserving the Dorsal Keystone Area (DKA) – “bonny cap” with the same principles of the SRTa.
Thus, based on rational anatomical basis, and the acquired experience with SRTa, the author described a Cartilaginous Push-Down with Preservation of the Bony Cap, . This new technique named “Spare Roof Technique B” or “Ferreira-Ishida Technique” preserves the bony cap and ensures intact brow-tip dorsal aesthetic lines.
Since SRTb is a Surface Preservation Technique is allows the traditional structured manoeuvres like L-Shape septoplasty and regular lateral osteotomies to thinner the bony bridge, without impaction osteomies (traditional push/let down).
Expanding the indication the author will explain how to deal with the crooked nose.
Recently the author starts the rSRT – Reverse SRT – already accepted for publication – with application on the saddle nose and in augmentation rhinoplasty.
